Superior Mobile Lab and Courier Services

Welcome to Superior Mobile Lab and Courier Services ("Company", "we", "our", or "us"). By accessing our services—including but not limited to mobile phlebotomy, specimen collection, laboratory courier services, and related health logistics—you agree to be bound by these Terms and Conditions ("Terms"). Please read them carefully before engaging with our services.

We offer the following services:

• Mobile phlebotomy and specimen collection
• Delivery of biological specimens to laboratories or healthcare providers
• Mobile health screening and early gender reveal testing
• Specialty kit collection and return services
• Same-day and STAT delivery options

Services are available to individuals, healthcare providers, laboratories, and facilities within our designated service area. Service outside of this area may incur additional charges.

To use our services:

• You must be at least 18 years old or have a parent/legal guardian consent if under 18
• You must provide accurate and complete information when scheduling
• You must have valid lab orders if requesting diagnostic specimen collection

We reserve the right to refuse service to any person or entity for any reason at our discretion.

• Appointments must be scheduled in advance by phone or online at [www.superiormobilelabs.com].
• Appointments canceled with less than 24 hours' notice may be subject to a $25 cancellation fee.
• No-shows may result in a full service charge and potential refusal of future service.

• Mobile draw base fee: $75
• Additional kits: $ price varies
• Mileage fee: $0.70/mile outside of a 15-mile radius
• STAT or expedited services may incur additional fees

Payment is due at the time of service unless prior arrangements are made. We accept cash, credit/debit cards, HSA/FSA cards, and select digital payments. Returned payments may result in a $35 returned payment fee.

For specimen pickup and delivery:

• Proper packaging and labeling are required before pickup
• Specimens must comply with all applicable local, state, and federal guidelines
• The company is not liable for improperly packed specimens or delays caused by third-party labs

Pickup and delivery windows are estimates and subject to change due to traffic, weather, or operational delays.

• Provide safe, clean, and accessible collection environments (for mobile services)
• Ensure all lab requisitions and test kits are complete and valid
• Disclose any known bloodborne illnesses, infections, or conditions that may affect specimen collection
• Ensure availability during the scheduled appointment window

We are fully HIPAA-compliant and committed to safeguarding your protected health information (PHI). Your personal information will never be sold or shared outside the scope of the services provided without your written consent, unless required by law.

• We are not responsible for delayed lab results or errors made by third-party laboratories.
• We are not liable for adverse reactions or complications following blood draws or sample collections.
• We are not liable for missed diagnoses or decisions made based on test results.

Clients assume full responsibility for verifying their healthcare provider's instructions and laboratory orders.

We reserve the right to refuse service if:

• The environment is deemed unsafe
• The patient is uncooperative or abusive
• Improper or incomplete documentation is presented

No refund will be issued for refusal due to patient non-compliance or misrepresentation.

All content, branding, images, text, and digital materials created or provided by Superior Mobile Lab and Courier Services are protected under intellectual property law and may not be copied, distributed, or reused without express written consent.

We may update these Terms and Conditions from time to time. Changes will be posted on our website and are effective immediately upon posting. Continued use of our services after changes signifies acceptance of the new Terms.

These Terms are governed by the laws of the State of Florida. Any disputes will be resolved through arbitration or small claims court located in Highlands County, Florida.

Our team is trained in medical logistics and committed to maintaining the integrity of every specimen and document we transport.

• Time-sensitive STAT specimens
